I am aware that the school can only authorise absences in exceptional circumstances. I am also aware that taking my child out of school without permission during term time may result in a Fixed Penalty Notice (fine) being issued as outlined in the Lighthouse Schools Partnership Attendance Policy.*I confirm I have read the statement above and wish to submit this exceptional absence request form.

The School can only accept absence requests from those who have parental responsibility and I am the parent/carer with whom this child usually resides.*I confirm that I have parental responsbility as required to make this request
